CARE Forms

Find forms and flyers available to CARE students.

Make sure to check EOPS Forms regularly to stay up to date with important EOPS forms and deadlines. 

Below are various forms and flyers CARE students should access on a regular basis. Read the description for each document to help you select the correct document(s). If you have any questions or need a hard copies of any of the documents below, please feel free to contact or stop by the CARE office.

CARE Certification Form

New and continuing students must complete the CARE Agency Certification process each academic year (beginning July 1). There are two steps to the process:

  • Student completes the CARE Certification Form accepted until October 31 for fall 2021. Students are recommended to submit by September 3 to start processing for CARE services and grant.
  • Once the online eligibility form is completed, you will be contacted by a CARE staff member to go over your eligibility for the program and begin the intake process if all requirements are meet.

All CARE students have access to supplemental resources throughout the semester. Review the Important Dates and Resources flyer to learn how and when you can receive CARE services.

CARE Workshops

Every month this semester, CARE will be hosting Tea with Bree - a digital “hang out” with other parenting students - along with Office Hours, where students can come and ask quick questions.

RSVP for as many of each - no penalty if you do not attend; just support it you do. Additionally, CARE staff promote workshops hosted by campus and community partners. Check your ECC email and/or texts for more information.

Harriett Buhai Center for Family Law

All interested El Camino College students must complete a pre-screening form that is submitted to Breeanna Bond (bbond@elcamino.edu) and a follow-up call from Harriett Buhai Center Staff within five business days.

If you already have a case open with the Harriett Buhai Center, please contact them directly for case assistance.
